Volunteer Authorization *

I understand (and parent/guardian) I am requesting to be a volunteer for The Brave Warrior Project, and submitting this application does not automatically register me as a volunteer. I authorize The Brave Warrior Project to receive information from any law enforcement agency, including policy departments and sheriff’s departments, of this state or any other state or federal government, to the extent permitted by state and federal law, pertaining to any convictions I any have had for violations of state or federal criminal laws, including but not limited to convictions for crimes committed upon children or animals. I understand that such access is for the purpose of considering my applications an employee/volunteer, and I expressly DO NOT authorize The Brave Warriors Project officers, employees, or other volunteers to disseminate this information in any way to any other individual, group, agency, organization or corporation. In case of emergency, I give (and parent/guardian) permission to The Brave Warrior Project to secure medical treatment including x- ray, surgery, hospitalization and medication. Confidentiality Agreement: I understand (and parent/guardian) that all information (written and verbal) about participants at The Brave Warrior Project is confidential and will not be shared with anyone without the expressed written consent of the participants and their parent/guardian in the case of a minor. However, you are free to celebrate the successes of our Brave Warriors with your friends and families, just refrain from using their name.
